I would like to change my sons name in the state of Louisiana. I do not know where his father is.?

His father hasnt paid child support in over a year and has made no attempt to see or talk to my son for 2 1/2 years. How do I file for a change of name when I have no idea where his father lives? He has been a resident of LA, AL, GA and Mexico. I would like to give my son my last name. Can I file the paperwork myself?I would like to change my sons name in the state of Louisiana.
